Ameristar Home Healthcare is seeking a compassionate, reliable, and experienced Registered Nurse (RN) to join our home health team.
We are looking for a motivated nurse who enjoys providing quality patient care, working independently, and building positive relationships with patients and families.

Key Responsibilities
- Conduct Start of Care (SOC), Resumption of Care (ROC), and recertification assessments.
- Complete OASIS assessments accurately and timely.
- Perform weekly skilled nursing visits to assess patient conditions, monitor progress, and provide ongoing nursing care.
- Provide medication management, wound care, patient education, and other skilled nursing services as ordered.
- Develop, review, and update individualized patient care plans.
- Complete nursing visit notes and clinical documentation within agency deadlines.
- Communicate with physicians and the Director of Nursing regarding patient progress and changes in condition.
- Conduct supervisory visits for home health aides as assigned.
- Coordinate patient care with other healthcare professionals.
- Ensure compliance with agency policies and applicable state and federal regulations.
Qualifications
- Active Ohio Registered Nurse (RN) license.
- Minimum one year of nursing experience preferred.
- Previous home health nursing experience strongly preferred.
- Experience with OASIS assessments, SOCs, and recertifications preferred.
- Strong clinical assessment and documentation skills.
- Ability to work independently and manage assigned visits.
- Reliable transportation.
